The position of Community Life Coordinator provides case coordination for individuals who attend one of our day services programs--Rosewater, Inc. with the focus being on community involvement and meaningful day activities. Job duties include, but are not limited to:

  • Conducting one-on-one assessments with individuals to determine areas of interest and opportunity within the community.
  • Introducing individuals to community activities not experienced previously in order to broaden their experiences and perspective.
  • Exploring employment opportunities in the community and advocating for individuals with potential employers and others.
  • Working with individuals to develop skills needed to make community employment both possible and successful.
  • Providing oversight to individuals working in community jobs to ensure their continued success. Act as a resource to employers if issues arise in the workplace.
  • Planning Meaningful Day Activities and ensuring their completion for all individuals on your caseload.
  • Providing supervision and oversight of and serving as Mentor, Trainer, and Coach to Direct Support personnel working with individuals on your caseload.
  • Interviewing, hiring, scheduling, and performing other supervisory duties related to the Direct Support Professionals working with your caseload.
  • Serving as a member of the Interdisciplinary team and attending all Person Centered Planning meetings/Case Conferences. Give input regarding recreational, vocational, and educational opportunities.
  • Providing thorough, accurate, timely, and professional Case Notes, Progress Notes, and other reports as required.
  • Assume on-call responsibilities in rotation with other professionals.


**Requires BA/BS and previous case management experience. Previous supervisory experience is helpful.
