The position of Registered Nurse performs Health Care coordination for a caseload of individuals residing in residential settings in one of the following areas: South Bend, Valparaiso, Fort Wayne, Indianapolis/Shelbyville, or Atlanta. The RN ensures each individual has optimal health by: - Maintaining complete and accurate charts in accordance with State and Federal guidelines for ICFs/MR
- Performing quarterly and PRN healthcare assessments
- Educating Direct Support staff (Mentors) in Core A and Core B--the official State class that enables Direct Support personnel to pass oral and topical medications and give treatments.
- Directly observing Mentors during med passes to ensure error-free medication administration
- Providing information and education about medication effects and side effects
- Acting as a liaison between physicians, families, pharmacy, Mentors, and individuals in regards to health care
- Coordinating medical and dental appointments as required by State regulations or PRN and completing Nursing Notes for each visit (or issue, even if a doctor visit is not the result)
- Taking any new physician's orders
- Phoning in new orders to pharmacy
- Ensuring correct medications are delivered by pharmacy
- Arranging quarterly pharmacy reviews
- Reviewing new MAR sheets monthly to ensure accuracy
- Serving as a member of and/or presenting information to the Clinical Review Committee and Human Rights Committees as required by Company and State guidelines
- Spending at least 25% of work hours in the residential locations before and after typical business hours
BS in Nursing and current Indiana Nursing license required. Computer Savvy, Self-Starter, Strong teaching skills
